IMPORTANT - READ BEFORE SIGNING:  I/We, the parents/guardians of the above named candidate for a position on a Farmers Branch Youth football or Cheerleading team, do hereby give my/our approval and consent to participate in any and all Farmers Branch Youth Football and Cheerleading Association activities.  I/we, for ourselves and on behalf of the above-named candidate, do hereby agree to assume all risks and hazards incidental to such participation in said football program, including, but not limited to, the playing of football, cheerleading, the practicing of football or cheerleading, and the transportation to and from the activities; and I/we do hereby waive, release, absolve, indemnify, and agree to hold harmless the league’s organizers, sponsors, supervisors, participants, and persons transporting my/our child to & from the activities for any claim arising out of an injury to my/our child, whether the result of negligence or from any other cause, except to the extent and amount covered by an accident or liability insurance policy in force at the time of injury.  I/we agree to return, upon request, the uniform and other equipment issued to my/our child in as good a condition as when received except for normal wear and tear.  I/we will furnish a certified birth certificate of the above-named candidate to League officials.  I/we understand the registration fee, used for equipment and uniform purchase, is nonrefundable.

Parents will be required to sign the Parent’s Code of Ethics at registration.

We ask that all parents volunteer 2 hours of their time to the league during the season.  

As a Parent /Guardian, registering a child, you become a member of the FBYFCA.   Members will have an orientation session at the beginning of the season and elect the directors of the association at the annual season ending event.

 

Note: Each team is limited to 25 players. The first 25 players to clear registration will make the team, this includes complete application, birth certificate, all fees and/or scholarship forms.  NO EQUIPMENT OR UNIFORMS WILL BE PASSED OUT UNTIL FEES ARE PAID.   Football equipment must be returned at the end of the season.  This includes  – helmet, shoulder pads, 7 piece pads & game pants.  $25 is returned to tackle players from registration fee when this obligation is met at the end of the season.  Flag players must only return game pants.  No part of registration fee is returned to Flag players.

If registering before 5/31/08, take $10 discount off of fully paid registration fee.  Families with multiple children participating will receive $10 off each additional child. $25 fee will be charged on all returned checks.
